How referrals work
A clear, respectful process
Our goal is to make referrals simple for partners and supportive for the person being referred. If you’re unsure whether someone is a fit, reach out—we’re happy to talk it through.
1) Reach out
Send a referral inquiry with basic details and the best way to follow up.
2) Eligibility & next steps
We review needs, safety considerations, and program expectations, then outline next steps.
3) Intake conversation
We connect with the individual (when appropriate) to understand goals, supports, and readiness.
4) Placement & support plan
If accepted, we coordinate move-in timing and begin a structured support plan focused on stability and independence.

Who can make a referral?
Individuals, family members, and community partners (case managers, outreach teams, hospitals, shelters, and other providers) are welcome to reach out.
Do you require consent from the individual?
Consent is strongly preferred. If consent isn’t possible, contact us and we’ll discuss what can be shared and how we can proceed respectfully.
What does “supportive housing” mean in your program?
Residents have a safe place to live while working toward stability with structured support, routine-building, and connections to community resources.
How long does the process take?
Timelines vary based on availability and the information provided. We aim to respond promptly and keep partners informed of next steps.
What if someone isn’t a fit right now?
We’ll share what we can and, when possible, suggest alternative resources or next steps to support the person’s safety and stability.
Is there a cost to participate?
